Dear OneTouch® Customer:
Our company is committed to improving the quality of life for people with diabetes. We hold our products to the highest standards of quality, constantly working to ensure they provide glucose results you can rely on. For that reason, we're also committed to communicating with you when we learn of product that does not meet our expected standards.
We have determined that a very small number of OneTouch® UltraSmart® Meters recently distributed in Canada are set to the incorrect unit of measure (mg/dL) instead of the correct unit of measure used in Canada (mmol/L). Your test result will include a decimal point if it is set to the correct unit of measure (mmol/L).
Glucose readings shown in “mg/dL” are 18 times greater than readings shown in “mmol/L” (see Figures 1 and 2). For example, a result of 5.8 mmol/L would be shown as 104 mg/dL. This could lead to a misunderstanding of your test results and might lead to improper treatment.
Meters Set To The Wrong Unit Of Measure Will Be Replaced Free Of Charge
1. Please compare the serial number on the back of your OneTouch® UltraSmart® Meter (see Figure 3) with the list of affected products below.

Note: Only OneTouch® UltraSmart® Meters with the above serial numbers require replacement.
2. If your meter’s serial number matches one of the listed numbers, please call the OneTouch® Customer Care Line at 1-800-663-5521, Monday to Friday, 6:00am – 5:00pm PT (9:00am – 8:00pm ET) to receive a replacement meter free of charge.
